Notice of Privacy Practices

FastRad LLC ("FastRad," "we," "us," or "our") is required by law to maintain the privacy of your Protected Health Information (PHI) and to provide you with this Notice of our legal duties and privacy practices with respect to your PHI.
We are required to follow the terms of this Notice currently in effect. This Notice applies to all records of your care generated by FastRad.
What is Protected Health Information (PHI)?
PHI is information about you, including demographic information, that may identify you and that relates to your past, present, or future physical or mental health or condition and related healthcare services.
How We May Use and Disclose Your PHI
1. For Treatment
We may use and disclose your PHI to provide, coordinate, or manage your healthcare. This includes:
- Performing diagnostic imaging procedures ordered by your physician
- Providing radiologist interpretation of your images
- Sharing results with your referring physician and healthcare team
- Communicating with other healthcare providers involved in your care
Example: We will send your X-ray results to the physician who ordered the test so they can determine the appropriate treatment for your condition.
2. For Payment
We may use and disclose your PHI to obtain payment for services. This includes:
- Billing you, your insurance company, or third parties for services rendered
- Verifying insurance coverage and benefits
- Processing insurance claims
- Collection activities for outstanding balances
Example: We will submit a claim to your insurance company that includes information about the imaging service provided and the diagnosis from your physician's order.
3. For Healthcare Operations
We may use and disclose your PHI for our healthcare operations, including:
- Quality assessment and improvement activities
- Staff training and education
- Accreditation, certification, and licensing
- Business planning and development
- Conducting or arranging for medical reviews, legal services, and audits
Example: We may review imaging studies to ensure our radiologists are providing accurate interpretations and to improve service quality.
Other Uses and Disclosures That Do Not Require Your Authorization
We may use or disclose your PHI without your authorization in the following situations:
- As Required by Law: When required by federal, state, or local law
- Public Health Activities: To prevent or control disease, injury, or disability; to report births, deaths, or suspected abuse/neglect
- Health Oversight Activities: To health oversight agencies for audits, investigations, inspections, or licensing
- Judicial and Administrative Proceedings: In response to a court order, subpoena, or other lawful process
- Law Enforcement: For law enforcement purposes in limited circumstances
- Serious Threat to Health or Safety: When necessary to prevent a serious threat to your health and safety or that of the public
- Workers' Compensation: For workers' compensation or similar programs
- Military and Veterans: If you are a member of the armed forces
- National Security: For national security and intelligence activities
- Coroners and Medical Examiners: To identify a deceased person or determine cause of death
Uses and Disclosures That Require Your Written Authorization
Other uses and disclosures not described in this Notice will be made only with your written authorization, including:
- Marketing purposes
- Sale of PHI
- Psychotherapy notes (if applicable)
- Other uses not described in this Notice
You have the right to revoke your authorization at any time by submitting a written revocation. The revocation will not affect disclosures already made in reliance on your authorization.
Your Rights Regarding Your PHI
1. Right to Request Restrictions
You have the right to request restrictions on how we use or disclose your PHI for treatment, payment, or healthcare operations. We are not required to agree to your request, except in the case of disclosures to a health plan for payment or healthcare operations when you have paid out-of-pocket in full.
2. Right to Receive Confidential Communications
You have the right to request that we communicate with you about your health matters in a certain way or at a certain location. For example, you may request that we contact you at work instead of home.
3. Right to Inspect and Copy
You have the right to inspect and obtain a copy of your PHI contained in your medical and billing records. We may charge a reasonable fee for copying and mailing costs. We may deny your request in certain limited circumstances.
4. Right to Amend
You have the right to request an amendment to your PHI if you believe it is incorrect or incomplete. We may deny your request if the information was not created by us, is not part of our records, or is accurate and complete.
5. Right to an Accounting of Disclosures
You have the right to receive a list of certain disclosures we have made of your PHI during the six years prior to your request (excluding disclosures for treatment, payment, healthcare operations, and certain other exceptions).
6. Right to a Paper Copy of This Notice
You have the right to receive a paper copy of this Notice upon request, even if you have agreed to receive it electronically.
7. Right to Be Notified of a Breach
You have the right to be notified if your unsecured PHI is breached.

Changes to This Notice
We reserve the right to change this Notice at any time. We reserve the right to make the revised Notice effective for PHI we already have as well as any PHI we create or receive in the future.
We will post a copy of the current Notice on our website. The Notice will contain the effective date on the first page.
